PacMed is looking for a LPN to work in collaboration with other members of the health care team while integrating the nursing roles of clinical, teaching, consulting, care coordination and case management.

In this position you will:

  • Assesses patient needs, develops a plan of care, delivers direct care, evaluates outcome of care, and adjusts plan and subsequent interventions accordingly

  • Administers medications within the scope of practical nursing practice

  • Documents subjective and objective assessment, evaluation, plan and interventions for in-person and telephone interactions

  • Conducts comprehensive educational assessments, matches teaching methods to defined learning objectives, and develops teaching plans for individual patients and target population groups.

  • Coaches, instructs and acts as a role model for designated health care team workers

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited practical nursing program.

  • Basic Life Support certification within 2 month of hire, required.

  • Current LPN license in Washington State.

  • 1 year experience in a patient care setting.

Preferred qualifications for this position include:

  • 2 years LPN or MA experience in a patient care setting.

  • Recent experience in an out-patient setting.
