Pacific Medical Centers Director of Population Health Management & Quality - Accountable Care in Seattle, Washington

We are seeking a Director of Population Health Management & Quality to provide leadership and oversight, develop, direct and manage PacMed’s Case Management department and associated staff, monitor inpatient and outpatient Case Management systems, and to interface with the QCCC.

In this position you will:

  • Report to the COO

  • Direct, Manage, and supervise staff in accomplishment of the goal of the organization.

  • Develop and be responsible for the PMC program for meeting Risk contract standards regarding care management, Utilization Management, delegation status and NCQA standards for medical review.

  • Recruit, hire, and supervise professional support staff; orient, train, and support staff in performance of their duties; evaluate performance and provide career development coaching.

  • Work closely with Quality Analyst to monitor and develop targets and analyze actual performance data against them; share responsibility with manager for specified month-end reports; collaborate in development of formats for standard and specialized reports; assists with collection and analysis of outpatient data.

  • Participate in Contract utilization and provider meetings.

  • Assist with the monitoring, managing an ongoing evaluation of contract compliance; assist with education of outside contracted providers; recommend new opportunities for outside vendors as alternative to hospital based care.


Required qualifications for this position include:

  • Degree in nursing, social work or other applicable health care degree.

  • CCM certification.

  • Minimum five (5) years clinical nursing or social work or other applicable healthcare experience.

  • Minimum (2) years Management, including experience with Referral management, case management, contract review, and other applicable experience needed to manage populations.

  • Experience designing and managing processes.

  • Supervisory and management experience

  • Light travel, mostly locally, required
