The Health Information Management Specialist will maintain and update patient records, which include medical histories, treatments, and examinations. Responsible for maintaining, filing, storing paper records and facilitates the use of Pacific Medical Centers' paper-based and electronic medical records, Provides assistance to providers, staff, and patients for the maintenance and release of ambulatory medical records. Consistently follows HIM policies and procedures and contributes to the efficiency of the clinic and health information services.
In this position you will:
Sort all incoming papers, received from the clinics by interoffice, Rightfax/ Fax machine, and email which includes date stamp incoming documents, checking for two patient identifiers on each document and checking for accuracy of correct medical record to prepare for scanning or sending to clinics for provider review
Scan medical records into Epic, using the established Scanning guidelines
Monitor incoming and scanned documents to ensure accuracy of the data being input into the EMR
Follow up with the provider and/or Supervisor as necessary to resolve any found issues.
Process ROI requests; follows protocols of release of information program
Scan or photocopies patient health information for release to requester/recipient on a STAT basis
Provide customer services for in-person and telephone requests, faxing information in a timely accurate manner. Process patient direct request for medical records to self/other. Coordinating with the patient for processing needs
Receive and handle calls from multiple phone lines from patients, providers, other departments, copying firms, insurance companies, and outside clinics, providers and business partners
Merge charts and/or medical record numbers for duplicate medical record number assignments
Maintain the integrity and confidentiality of the medical record following State, Federal and HIPAA guidelines
Operate equipment, including: telephones, photocopier, and FAX machine
Process MyChart Proxy account access
Workgroup for Death Notification: EDRS - PMC Providers
Required qualifications for this position include
Successful completion of a HIM-approved medical terminology program or class
Two (2) years of Health Information Department or equivalent education
Three (3) years of Customer service
