Pacific Medical Centers Population Health Coordinator - Accountable Care in Seattle, Washington

Find what you’re looking for in your career, at PacMed as a Population Health Coordinator (1.0 FTE, Day Shift) to work in Accountable Care in Seattle, WA.

Supports the population health team by entering/coordinating incoming referrals and tracking outreach. Makes outgoing calls/sends Mychart messages to schedule appointments, provide resources, coordinate care, and provide class and visit preparation instructions to patients. Answers incoming phone calls from patients, providers, clinic staff and resources. Compiles data and prepares/submits reports for program, care and organizational performance metrics. Schedules conference rooms/venues for education classes. Provides routine financial counseling, researches routine referral and billing issues, and completes registration and data entry for programs

In this position you will:

  • Monitors patient progress and challenges using information derived from reports and patient contact.

  • Enters referrals into system and advises patients on routine insurance and referral issues.

  • Researches and coordinates community and financial resources for patients in coordination with Care Managers and Supervisor.

  • Registers new patients, completing demographic and financial fields in the registration system, assigns financial codes, and may obtain proof of insurance. Verifies benefits and eligibility. Copies insurance cards and updates registration as needed

  • Contacts patients and gives them instructions on the prep necessary to complete the procedures, appointments, and participate in classes

  • Schedules procedures, routine pre-procedure diagnostic tests and interpreters as needed. Makes appointment reminder calls; processes routine referrals for authorization; ensures Appointment Summary screens reflect the day’s activities.

  • Schedules conference rooms or other location for classes and is responsible for set up of all equipment, furniture and complete all other tasks necessary for the class to take place

Qualifications:

Required qualifications for this position include:

  • High School Diploma or equivalent.

  • Valid Washington State Driver’s License and current auto insurance.

  • 2 years Health care experience in ambulatory practice.

  • 2 years driving experience

Preferred qualifications for this position include:

  • Associate's Degree

  • Prior experience using Excel spreadsheets.

  • One year experience with coding: ICD-9 or ICD-10, CPT, HCPCS.

  • Prior experience with an EMR.

  • Prior experience in liaison role for patients, providers and staff.
