Pacific Medical Centers Population Health Coordinator - Accountable Care in Seattle, Washington

We are seeking a Population Health Coordinator to coordinate incoming referrals, care follow-up documentation and data to departments for appointments, resources, additional care options, transitional care, and other care needs.

In this position you will:

  • Enter referrals into system and advise patients on routine insurance and referral issues.

  • Research and coordinate community and financial resources for patients in coordination with Care Managers and Supervisor.

  • Prepare insurance forms and respond to insurance requests.

  • Code write-in diagnoses and procedures.

  • Prepare data for Care managers to utilize in patient care planning and coordination and outreach.

  • Register new patients, completing demographic and financial fields in the registration system, assigns financial codes, and may obtain proof of insurance.

  • Copy insurance cards and updates registration as needed.


Required qualifications for this position include:

  • High school diploma or equivalent.

  • 2 year’s health care experience in ambulatory practice

  • Mimium of 2 years of driving expereince

  • Valid Washington State Driver's License and current auto insurance

Preferred qualifications for this position include:

  • Bachelor’s degree.

  • One year patient third party billing and referrals experience on an automated patient accounts system.

  • Prior experience in liaison role for patients, providers and staff.

  • Comprehensive working knowledge of third party insurance.
