This is a full time position and is eligible for full benefits! This position will work at Beacon Hill, First Hill and Northgate PacMed Clinics.

We are seeking a RN Care Manager to coordinate care for Pacific Medical Centers patients (excluding USFHP) in order to assure cost effective and appropriate utilization of health care resources.

In this position you will:

  • Create and recommend care plans for high risk patients in the outpatient setting including communication with physicians, nurses, homecare providers or others as applicable.

  • Collaborate with the physician or provider regarding appropriate disposition of patients from hospital or specialized nursing facility.

  • Educate and direct providers both within and outside the Pacific Medical system regarding benefits and utilization of services.

  • Direct Case Managers from outside facilities regarding: use of benefits and discharge planning options; consults with providers regarding complex cases and discusses appropriateness of hospitalization or care in skilled nursing facility.

  • Act as a resource for clinical care plans; Risk Contracts/ACO and Medicare benefits and guidelines to coordinate patient care; advises patients and families regarding benefits, limitations, and appropriate use of resources.

  • Be responsible for managing complex patients in the outpatient setting utilizing homecare, hospice, DME and other appropriate agencies.

Qualifications:

Required qualifications for this position include:

  • Current Washington State Registered Nurse License.

  • BLS or CPR certification within 1 month of hire.

  • Five (5) years clinical experience.

  • Previous experience in utilization review/case management.

Preferred qualifications for this position include:

  • BSN.

  • Certified Case Manager.
