We are seeking a Social Worker to be responsible for providing case coordination and a full range of social work services for non-medical issues, including psychosocial assessment, treatment planning, utilization management, therapeutic interventions, discharge planning, crisis intervention, referral and resource.
In this position you will:
Provides direct mental health services to adults, families, and children within the scope of training, including individual, family, marital, and group counseling; clinical model should emphasize the efficient use of treatment through focused, brief interventions that build upon clients’ existing strengths and improve coping skills.
Works closely with intake coordinator to quickly bring new patients into treatment; prioritizes time to assure that most pressing cases are carefully attended to and balances overall caseload.
Works closely with the case management process in the authorization of care.
Maintains appropriate written records of treatment in patient medical record, encounter form, and authorized release of information; assures that other providers involved with the patient have timely information to provide the best care.
Involves him/herself in clinical supervision provided within the department and initiates appropriate consultation with other providers when cases so require.
Maintains professional license by meeting all requirements on an ongoing basis.
Required qualifications for this position include:
Master's Degree Social Work, from an accredited school.
Coursework/Training Specific training in the provision of outpatient community services.
Coursework/Training Training in using efficient, focused treatment approaches to patient problems.
Specific experience in the provision of outpatient community services.
Experience in using efficient, focused treatment approaches to patient problems.
Licensed, registered, or certified by the State of Washington as required for that discipline.
