Find what you’re looking for in your career, at PacMed as a Social Worker MSW Population Health Care (1.0 FTE, Day Shift) at the Accountable Care Department in Seattle, WA.
We are seeking a Social Worker to be responsible for providing case coordination and a full range of social work services for non-medical issues, including psychosocial assessment, treatment planning, utilization management, therapeutic interventions, discharge planning, crisis intervention, referral and resource.
In this position you will:
Provide direct social health services to patients within the scope of training, including individual, family, marital, and group counseling and resource assistance.
Work closely with patient care coordinators and others to quickly bring patients into treatment; prioritizes time to assure that most pressing cases are carefully attended to and balances overall caseload.
Work closely with the case management process.
Maintain appropriate written records of treatment in patient medical record, encounter form, and authorized release of information; assure that other providers involved with the patient have timely information to provide the best care.
Work closely with the clinic care teams, community resources, practitioners, administration, and other stake holders to assure that the Pacific Medical Center (PMC) patient population’s needs are met.
Maintain professional license by meeting all requirements on an ongoing basis.
Closely follow the financial practices of PMC, including assuring that services are fairly covered by patient’s health plan and/or that clear financial arrangements have been made with the patient regarding services to be provided.
Required qualifications for this position include:
Master's Degree in Social Work from an accredited school
Specific Training in the provision of outpatient community services.
Training in using efficient, focused treatment approaches to patient problems.
Licensed, registered, or certified by the State of Washington as required for that discipline.
Preferred qualifications for this position include:
Knowledge of Electronic Medical Record
Computer use skills, including email, required. Outlook, MS Word and Excel
