The Social Worker is responsible for providing case coordination and a full range of social work services for non-medical issues, including psychosocial assessment, treatment planning, utilization management, therapeutic interventions, discharge planning, crisis intervention, referral and resource, and acts as a liaison between USFHP Medical Director, Director of Operations, Case Managers, members and Medical providers including Skilled Nursing Attending.
In this position you will:
Works closely with Nurse Case Managers, Medical Director and Skilled Nursing Attending in providing case coordination and direct care services to USFHP members and their families; services including care conferences, discharge planning, utilization management, crisis intervention, referral and resource. Works within a clinical model that emphasizes the efficient use of treatment through focused, brief interventions that build upon clients’ existing strengths and improve quality of life.
Works closely with the case management team in the authorization of care.
Maintains patient confidentiality as required by law and professional ethics. Maintains appropriate written records of treatment in electronic medical record and authorized release of information forms. Ensures that other providers involved with the patient have timely information to provide the best care.
Sets work hours and practice patterns in agreement with the Director of USFHP and the Manager of Care Coordination.
Stays involved in the clinical supervision provided within the department and initiates appropriate consultation with other providers when required.
Required qualifications for this position include:
Bachelor's Degree in Social Work, from an accredited school
Training in using efficient, focused treatment approaches to patient problems.
5 years of related experience
Experience in the provision of outpatient community services and discharge planning.
Preferred qualifications for this position include:
Master's Degree in Social Work, from an accredited school
Licensed, registered, or certified by the State of Washington as required for that discipline preferred.
